Concord Lodge provides a residential assessment service supporting adults with learning difficulties and higher support needs. As part of the assessment we work closely with our health colleagues supporting a full medical assessment. Our aim is to assess and promote full community inclusion. To provide an in-depth assessment looking at daily living skills then identifying a full support package of care.
We are seeking a care officer to support service users through the assessment process. You will be joining a well-established team who are committed to promote good outcomes for service users.
As a Care Officer you will be supporting the key work team with the assessment process, promoting person centred approach. You will be a part of multi–professional meetings in identifying what assessments need to be worked on. You will be focused on supporting the individual service user from their admission through the assessment process. As part of the key work team you give direct support supporting service users and collecting information updating daily, also sharing the assessment to our in-house social worker. Once the assessment is completed you with the key worker team will identify the transition to the next appropriate placement/Service.
You will provide supervision to Care assistants. You must have good communication skills and work well within a team and as an individual.
You will support the team with all aspects of Health and safety.
